目的：探讨腹腔镜胆道探查术后胆管一期缝合与T管引流的疗效差异。方法：2013年2月-2014年3月收治胆囊结石合并胆总管结石患者80例，随机分成一期缝合组和 T 管引流组各40例，并采取相应的方法进行手术。结果：两组在手术时间、胆道镜取石时间、术后胆瘘、术后住院时间方面比较无统计学意义(P＞0.05)，但是在恢复正常生活时间方面，一期缝合组明显少于T管引流组，两组比较差异有统计学意义(P＜0.05)。结论：一期胆总管缝合是较为理想的手术方法，不仅可以缩短患者的康复时间，而且能提高患者临床疗效和生活质量，值得临床推广。

목적：탐토복강경담도탐사술후담관일기봉합여T관인류적료효차이。방법：2013년2월-2014년3월수치담낭결석합병담총관결석환자80례，수궤분성일기봉합조화 T 관인류조각40례，병채취상응적방법진행수술。결과：량조재수술시간、담도경취석시간、술후담루、술후주원시간방면비교무통계학의의(P＞0.05)，단시재회복정상생활시간방면，일기봉합조명현소우T관인류조，량조비교차이유통계학의의(P＜0.05)。결론：일기담총관봉합시교위이상적수술방법，불부가이축단환자적강복시간，이차능제고환자림상료효화생활질량，치득림상추엄。
Objective:To explore the curative effect difference of primary suture and T tube drainage after laparoscopic common bile duct exploration.Methods:80 cases with cholecystolithiasis and choledocholithiasis were selected from February 2013 to March 2014.They were randomly divided into the primary suture group and T tube drainage group with 40 cases in each.They took the corresponding method of operation.Results:There were no statistical significances of two groups with operation time, choledochoscopic stone extraction time,postoperative biliary fistula,postoperative hospitalization time(P>0.05).However,in the return to normal life time,the primary suture group was less than that of T tube drainage group,and the difference was statistically significant(P<0.05).Conclusion:Primary suture of common bile duct is an ideal operation method.It can not only shorten the recovery time of patients,but also can improve the clinical curative effect and the quality of life of patients.So it is worthy of clinical promotion.
