目的：探讨社区构建和实施糖尿病防治模式，并对其效果进行分析。方法：在社区构建糖尿病监控和健康教育系统，并在社区选取160例糖尿病患者作为研究对象，其中控制不佳80例，控制良好80例，对其进行护理干预和随访。对比两组干预前后糖尿病知识的了解程度、治疗达标率。结果：干预后两组糖尿病知识了解程度及自我管理能力，均有显著增强，且两组治疗达标率均有明显提高，具有统计学意义(P＜0.05)。结论：在构建社区糖尿病防治模式中加入糖尿病监控和健康教育系统，能有效规范社区糖尿病模式，提高防治水平，加强患者对糖尿病知识的了解程度，改善治疗达标率，从而提高社区糖尿病患者的生活质量。
목적：탐토사구구건화실시당뇨병방치모식，병대기효과진행분석。방법：재사구구건당뇨병감공화건강교육계통，병재사구선취160례당뇨병환자작위연구대상，기중공제불가80례，공제량호80례，대기진행호리간예화수방。대비량조간예전후당뇨병지식적료해정도、치료체표솔。결과：간예후량조당뇨병지식료해정도급자아관리능력，균유현저증강，차량조치료체표솔균유명현제고，구유통계학의의(P＜0.05)。결론：재구건사구당뇨병방치모식중가입당뇨병감공화건강교육계통，능유효규범사구당뇨병모식，제고방치수평，가강환자대당뇨병지식적료해정도，개선치료체표솔，종이제고사구당뇨병환자적생활질량。
Objective:To explore the construction and implementation of the diabetes prevention and cure mode in community. The effect was analyzed.Methods:The diabetes monitoring and health education system were constructed in community.160 cases with diabetes of community were selected as the research objects.80 cases were poorly controlled group.80 cases were good controlled group.The patients were given the nursing intervention and follow-up.The understanding degrees of diabetes knowledge and the treatment success rates of patients in two groups were compared before and after the intervention.Results:The understanding degrees of diabetes knowledge and the self-management skills of patients in two groups were significantly enhanced after the intervention.The treatment success rates of patients in two groups were significantly improved,and it has statistical significance(P<0.05).Conclusion:The diabetes monitoring and health education system were joined to the construction of the community diabetes prevention and cure mode.It can effectively regulate the community diabetes model,rise the level of prevention and cure,strengthen the understanding degree of diabetes knowledge of patients,and improve the treatment success rate,so as to improve the living quality of diabetes patients in community.
