目的：对外科手术治疗胸腺瘤的疗效进行探讨和分析。方法：2010年3月-2014年3月收治胸腺瘤患者35例，对其随访资料和临床资料进行回顾性分析。结果：35例胸腺瘤患者中单纯胸腺瘤20例，胸腺瘤合并重症肌无力15例；经传统的胸部开放手术26例，经胸腔镜手术9例，所有患者都完全切除肿瘤。手术中都进行纵膈及胸腺脂肪组织的清扫，所有患者在住院期间没有死亡，术后有3例患者发生肌无力危象，经过抢救后治愈，重症肌无力患者术后症状缓解。对患者术后3年进行随访发现，术后3年生存率达到90%，术后对所有患者进行CT复查发现局部占位性病变消失。结论：手术是目前治疗胸腺瘤的主要方法，疗效明显。对重症肌无力患者术后要警惕肌无力危象的发生，及早行气管切开是抢救肌无力危象的重要措施。

목적：대외과수술치료흉선류적료효진행탐토화분석。방법：2010년3월-2014년3월수치흉선류환자35례，대기수방자료화림상자료진행회고성분석。결과：35례흉선류환자중단순흉선류20례，흉선류합병중증기무력15례；경전통적흉부개방수술26례，경흉강경수술9례，소유환자도완전절제종류。수술중도진행종격급흉선지방조직적청소，소유환자재주원기간몰유사망，술후유3례환자발생기무력위상，경과창구후치유，중증기무력환자술후증상완해。대환자술후3년진행수방발현，술후3년생존솔체도90%，술후대소유환자진행CT복사발현국부점위성병변소실。결론：수술시목전치료흉선류적주요방법，료효명현。대중증기무력환자술후요경척기무력위상적발생，급조행기관절개시창구기무력위상적중요조시。
Objective:To explore and analyze the curative effect of surgical operation treatment of thymoma.Methods:35 cases with thymoma were selected from March 2010 to March 2014.The follow-up data and clinical data were retrospectively analyzed. Results:In 35 cases with thymoma,20 cases were pure thymoma;15 cases were thymoma combined with myasthenia gravis.26 cases had traditional chest open surgery;9 cases had thoracoscopic surgery.The tumors of all patients were resected totally.They were given the cleaning of mediastinal and thymus tissue during operation.All patients had no death during the hospital stay.After surgical treatment of all patients with thymoma,3 cases occurred myasthenia gravis crisis after operation,and they were cured after rescue.Postoperative symptoms of patients with myasthenia gravis were relieved.The patients were followed up postoperative 3 years,the survival rate of postoperative 3 years reached 90%.The CT review of all patients after operation showed that local space-occupying lesions disappeared.Conclusion:Operation is the main method of treating thymoma now.Its curative effect is obvious.It should alert the occurrence of myasthenia gravis crisis in the patients with myasthenia gravis.Early tracheotomy is an important measure to rescue myasthenia crisis.
