目的：探讨比较CT、MRI及脑血流灌注显像（ SPECT ）诊断缺血性脑卒中的价值。方法219例患者进行CT、MRI及SPECT检查，并结合临床神经功能评分评估三种检查手段的诊断价值。结果 SPECT显像对短暂性脑缺血发作和腔隙性脑梗死的诊断检出率高于MRI和CT；在多发脑梗死和大面积脑梗死诊断方面，SPECT和MRI之间检出率无显著性差异，但均高于 CT扫描。结论 SPECT与MRI进行图像融合后能明显提高缺血灶检出的灵敏度及准确率，在缺血性脑卒中的早期诊断和风险评估中具有重要意义。
목적：탐토비교CT、MRI급뇌혈류관주현상（ SPECT ）진단결혈성뇌졸중적개치。방법219례환자진행CT、MRI급SPECT검사，병결합림상신경공능평분평고삼충검사수단적진단개치。결과 SPECT현상대단잠성뇌결혈발작화강극성뇌경사적진단검출솔고우MRI화CT；재다발뇌경사화대면적뇌경사진단방면，SPECT화MRI지간검출솔무현저성차이，단균고우 CT소묘。결론 SPECT여MRI진행도상융합후능명현제고결혈조검출적령민도급준학솔，재결혈성뇌졸중적조기진단화풍험평고중구유중요의의。
Objective It is to explore and compare the values of MR , CT and cerebral blood flow perfusion imaging ( SPECT) for the diagnosis of ischemic stroke .Methods 219 cases of ischemic stroke patients were given CT , MRI and SECT examinations , and their diagnosis value was evaluated according with clinical nerve function scores .Results The detection rate of SPECT in the diagnosis of transient ischemic attack and lacunar cerebral infarction was higher than that of MRI and CT ;there was no significant difference in the rate between SPECT and MRI in the diagnosis of multiple cerebral infarction and mas -sive cerebral infarction , but their rates were both higher than that of CT .Conclusion SPECT combined with MRI after image infusion can obviously increase sensitivity and accuracy degrees of the ischemic focus , thus it has important significance in the early diagnosis and risk evaluation for ischemic stroke .
