目的：评价奥沙利铂和氟尿嘧啶经动脉介入化疗治疗消化道肿瘤导致的恶性腹水的安全性及有效性。方法将107例消化道肿瘤导致的恶性腹水患者随机分为对照组（n=47）采用奥沙利铂和氟尿嘧啶经腹腔灌注化疗和研究组（n=60）采用奥沙利铂和氟尿嘧啶经动脉灌注化疗，术后1月观察其疗效及不良反应发生情况。结果患者的腹水变化：对照组（n=46）显效3例、有效28例，研究组（n=59）显效23例、有效34例，研究组疗效明显优于对照组；余腹围变化情况、腹水脱落细胞阳性的转阴情况、腹水CEA及CA199阳性的变化情况、血CEA及CA199阳性的变化情况及患者生活质量评分变化，这些指标二组的差异均有统计学意义；而二组的不良反应差异无统计学意义。结论奥沙利铂和氟尿嘧啶经动脉灌注化疗的疗效优于腹腔灌注，同时二者的不良反应发生情况类似。当然，长期疗效仍需进一步的研究评估。

Objective To evaluate the safety and efficacy of oxaliplatin and fluorouracil arterial infusion chemotherapy in the treat-ment of malignant ascites caused by gastrointestinal tumors. Methods 107 cases of patients with malignant ascites caused by gas-trointestinal tumors were randomly divided into control group (n=47) using oxaliplatin and fluorouracil intraperitoneal chemotherapy and study group (n=60) using oxaliplatin and fluorouracil arterial infusion chemotherapy. 1 month after the operation, the efficacy and adverse events of both groups were observed. Results The changes of ascites of the patients:in the control group(n=46), signif-icant effective in 3 cases, effective in 28 cases and in the study group(n=59), significant effective in 23 cases, effective in 34 cases. The efficacy of the study group was significantly better than that of the control group. The differences of changes in abdominal girth, positive ascites exfoliated cells turning negative situation, ascites CEA and CA199 positive changes, blood CEA and CA199 positive changes and changes in quality of life scores in patients between two groups were statistically significant. The difference in adverse reactions between the two groups was not statistically significant. Conclusion The effect of oxaliplatin and fluorouracil arte-rial infusion chemotherapy is better than that of oxaliplatin and fluorouracil intraperitoneal chemotherapy, but the incidence of ad-verse reactions of both is similar. Of course, the long-term effects need to be further studied and evaluated.
