目的：探讨妊娠合并卵巢肿瘤诊疗中应用腹腔镜的安全性及可行性。方法：2009年1月-2013年12月收治妊娠合并卵巢肿瘤患者13例，行腹腔镜手术治疗作为腔镜组。选择同期开腹手术治疗妊娠合并卵巢肿瘤患者11例作为对照组。对两组的手术效果、并发症及随访妊娠结局进行分析探讨。结果：两组手术过程均顺利，腹腔镜组无中转开腹病例。腹腔镜组手术时间30～45 min，术中出血20～35 mL，术后排气时间1～1.5 d，术后住院时间3.5～4 d；开腹组手术时间35～60 min，术中出血40～80 mL，术后排气时间1～2 d，术后住院时间5～7 d，差异有统计学意义(P＜0.05)。24例均妊娠至足月，新生儿无窒息、畸形，妊娠结局差异无统计学意义(P＞0.05)。结论：腹腔镜下手术治疗妊娠合并卵巢良性肿瘤是安全可行的，出血少、创伤小、术后恢复快、对妊娠无不良影响，具有非常积极的应用价值。

목적：탐토임신합병란소종류진료중응용복강경적안전성급가행성。방법：2009년1월-2013년12월수치임신합병란소종류환자13례，행복강경수술치료작위강경조。선택동기개복수술치료임신합병란소종류환자11례작위대조조。대량조적수술효과、병발증급수방임신결국진행분석탐토。결과：량조수술과정균순리，복강경조무중전개복병례。복강경조수술시간30～45 min，술중출혈20～35 mL，술후배기시간1～1.5 d，술후주원시간3.5～4 d；개복조수술시간35～60 min，술중출혈40～80 mL，술후배기시간1～2 d，술후주원시간5～7 d，차이유통계학의의(P＜0.05)。24례균임신지족월，신생인무질식、기형，임신결국차이무통계학의의(P＞0.05)。결론：복강경하수술치료임신합병란소량성종류시안전가행적，출혈소、창상소、술후회복쾌、대임신무불량영향，구유비상적겁적응용개치。
Objective:To explore the safety and feasibility of laparoscopy in the diagnosis and treatment of pregnancy combined with ovarian tumor.Methods:13 patients with pregnancy combined with ovarian tumor were selected January 2009 to December 2013.They were given laparoscopy operative treatment as endoscope group.11 patients with pregnancy combined with ovarian tumor with laparotomy treatment were selected at same time as the control group.The operation effect,complication and pregnancy outcome of follow-up of two groups were analyzed and explored.Results:The operation processes of two groups were successful, and no case in the laparoscopic group was converted to laparotomy.The operation time in the laparoscopic group was 30~45 minutes;the hemorrhage during operation was 20~35 mL;the postoperative exhaust time was 1~1.5 days;the postoperative hospitalization time 3.5~4 days.The operation time in the laparotomy group was 35~60 minutes;the hemorrhage during operation was 40~80 mL;the postoperative exhaust time was 1~2 days;the postoperative hospitalization time 5~7 days.The difference was statistically significant(P<0.05).24 cases were pregnancy to mature.The differences of whether newborn asphyxia, deformity,pregnancy outcome was no statistically significant.Conclusion:Laparoscopic surgery for the treatment of pregnancy combined with ovarian benign tumor is safe and feasible,less bleeding,less trauma,faster postoperative recovery,no adverse effects on pregnancy.It has very positive application value.
