目的：探讨低位保肛手术治疗直肠癌的临床疗效。方法：2012年2月-2014年4月收治直肠癌患者132例，根据手术方式的不同分为低位保肛术组和miles组，各66例，低位保肛术组采取低位保肛手术方式进行治疗，miles组采用Miles术式进行治疗。结果：低位保肛术组术后出血量(117.6±20.5)mL，术后肛门排气时间(3.1±1.2)d，术后并发症4例，并发症发生率6.1%，住院时间(16.5±2.3)d。miles 组术后出血量(271.6±23.5)mL，术后肛门排气时间(2.9±1.1) d，术后并发症11例，并发症发生率16.7%，住院时间(25.5±5.5)d，两组患者在术后出血量、术后并发症和住院时间比较差异有统计学意义(P＜0.05)，而术后肛门排气时间比较差异无统计学意义(P＞0.05)。低位保肛术组患者在躯体化、敌对、抑郁、恐惧、焦虑方面评分明显低于miles组，两组比较差异有统计学意义(P＜0.05)。结论：低位保肛手术治疗直肠癌提高了患者的生活质量，而且有手术过程出血少、住院时间短、并发症少等优点，值得推广。

목적：탐토저위보항수술치료직장암적림상료효。방법：2012년2월-2014년4월수치직장암환자132례，근거수술방식적불동분위저위보항술조화miles조，각66례，저위보항술조채취저위보항수술방식진행치료，miles조채용Miles술식진행치료。결과：저위보항술조술후출혈량(117.6±20.5)mL，술후항문배기시간(3.1±1.2)d，술후병발증4례，병발증발생솔6.1%，주원시간(16.5±2.3)d。miles 조술후출혈량(271.6±23.5)mL，술후항문배기시간(2.9±1.1) d，술후병발증11례，병발증발생솔16.7%，주원시간(25.5±5.5)d，량조환자재술후출혈량、술후병발증화주원시간비교차이유통계학의의(P＜0.05)，이술후항문배기시간비교차이무통계학의의(P＞0.05)。저위보항술조환자재구체화、활대、억욱、공구、초필방면평분명현저우miles조，량조비교차이유통계학의의(P＜0.05)。결론：저위보항수술치료직장암제고료환자적생활질량，이차유수술과정출혈소、주원시간단、병발증소등우점，치득추엄。
Objective:To explore the clinical effect of low anus preserving operation in the treatment of rectal cancer.Methods:132 patients with rectal cancer were selected from February 2012 to April 2014.They were divided into the low anus preserving operation group and the miles group with 66 cases in each according to the different operation ways.The low anus preserving operation group were treated with low anus preserving operation.The miles group were treated with Miles surgical method.Results:The postoperative blood loss of the low anus preserving operation group was (117.6±20.5)mL;the postoperative anal exhaust time was (3.1±1.2)days;the postoperative complication was 4 cases;the complication incidence rate was 6.1%;the hospitalization time was (16.5±2.3)days.The postoperative blood loss of the miles group was (271.6±23.5)mL;the postoperative anal exhaust time was (2.9±1.1)days;the postoperative complication was 1 case;the complication incidence rate was 16.7%;the hospitalization time was (25.5±5.5)days.The postoperative blood loss,postoperative complication and hospitalization time of patients in two groups were compared with statistically significant(P<0.05).The postoperative anal exhaust time was compared without statistically significant(P>0.05).The somatization, hostility,depression,fear,anxiety scores of patients in the low anus preserving operation group were significantly lower than those of the miles group,and the two groups were compared with statistically significant(P<0.05).Conclusion:The low anus preserving operation in the treatment of rectal cancer improve the life quality of patients,and it has the advantages,such as less bleeding during operation process,short hospitalization time,less complications.It is worthy of promotion.
