目的：探讨24 h动态心电图监测对西藏高原地区患者心律失常、心肌缺血的诊断价值，了解高原环境对高原地区患者心电图改变的影响。方法：2010年1月-2013年3月收治从西藏高原来的世居和移居西藏的患者368例，对其进行24 h动态心电图监测。结果：368例24 h动态心电图监测结果统计分析显示，24 h最大心率202次/min，常常发生在白天活动、情绪激动等情况时；24 h最小心率31次/min，多发生在深夜安静休息时；共检出各种心律失常366例(99.5%)，其中房性早搏306例(83.61%)，居第2位；其次是室性早搏193例(52.73%)；房性心动过速74例(20.22%)，居第3位；窦性心动过缓51例(13.93%)，居第4位。ST-T改变检出209例，检出率56.79%，均为无症状性心肌缺血。结论：24 h动态心电图监测对检出和评价患者的心律失常具有独特的作用，并具有重要的临床意义。

목적：탐토24 h동태심전도감측대서장고원지구환자심률실상、심기결혈적진단개치，료해고원배경대고원지구환자심전도개변적영향。방법：2010년1월-2013년3월수치종서장고원래적세거화이거서장적환자368례，대기진행24 h동태심전도감측。결과：368례24 h동태심전도감측결과통계분석현시，24 h최대심솔202차/min，상상발생재백천활동、정서격동등정황시；24 h최소심솔31차/min，다발생재심야안정휴식시；공검출각충심률실상366례(99.5%)，기중방성조박306례(83.61%)，거제2위；기차시실성조박193례(52.73%)；방성심동과속74례(20.22%)，거제3위；두성심동과완51례(13.93%)，거제4위。ST-T개변검출209례，검출솔56.79%，균위무증상성심기결혈。결론：24 h동태심전도감측대검출화평개환자적심률실상구유독특적작용，병구유중요적림상의의。
Objective:To investigate the diagnosis value of 24 h dynamic ECG monitoring on the Tibet plateau region of patients with arrhythmia,myocardial ischemia,to understand the influence of plateau environment on changes of electrocardiogram in patients with plateau area.Methods:368 patients who from the Tibet plateau to the native and moved to Tibet were selected from January 2010 to March 2013.They were given 24 h dynamic electrocardiogram monitoring.Results:368 cases of 24 h ambulatory ECG monitoring results statistics analysis display,24 h maximum heart rate was 202/min,and often occured in the daytime activity and emotional situation;24 h minimum heart rate was 31/min,occurred in the middle of the night to rest and quiet;all kinds of arrhythmia were detected in 366 cases(99.5%),including 306 cases of atrial premature beat(83.61%),ranking the second place;the second is premature ventricular contractions in 193 cases(52.73%);atrial Heartbeat tachycardia 74 cases(20.22%),ranking the third place;sinus Beckoning bradycardia in 51 cases(13.93%),ranking the fourth place.The ST-T changed were detected in 209 cases,the detection rate was 56.79% ,and all of them were asymptomatic myocardial ischemia.Conclusion:24 h dynamic ECG monitoring has a unique effect on the detection and evaluation of patients with arrhythmia,and it has important clinical significance.
