目的:对16层螺旋CT多平面﹑曲面重建及仿真内窥镜诊断气管异物的价值进行分析研究。方法选取2012年2月-2014年4月该院收治的经临床手术及纤维支气管镜证实的40例气管支气管异物患者,所有患者具有完整的CT资料,对其进行回顾性分析,将CT诊断结果与纤维支气管镜及临床手术情况做对比。结果该组研究中CT诊断的符合率为97.5%,CT直接征象包括气管﹑支气管腔内等/高密度影,管腔狭窄﹑阻塞,异物形态可见管状﹑柱状﹑环形﹑半圆形﹑类三角形及不规则形;间接征象可见胸腔积液﹑皮下气肿﹑纵隔气肿﹑肺炎﹑肺不张﹑肺气肿﹑气胸。结论16层螺旋CT多平面﹑曲面重建及仿真内窥镜在诊断气管异物方面确诊率较高,具有非常积极的应用价值。

Objective To analyze and study the value of 16-slice CT MPR, CPR and VE in the diagnosis of tracheal foreign body. Methods 40 cases with tracheal foreign body confirmed by clinical surgery and fiberoptic bronchoscopy in our hospital from February 2012 to April 2014 were selected. The complete CT data of all the patients were analyzed retrospectively. And the find-ings by CT were compared with the results of fiberoptic bronchoscopy and clinical surgery. Results In this study, the diagnostic ac-cordance rate of CT was 97.5%, the direct signs of CT included trachea, bronchus lumen etc./high density shadow, luminal steno-sis, occlusion, the foreign body presented as tubular, columnar, annular, semicircular, triangular or irregular shape; the indirect signs of CT were pleural effusion, subcutaneous emphysema, mediastinal emphysema, pneumonia, atelectasis, emphysema, pneu-mothorax. Conclusion 16-slice CT MPR, CPR and VE have a higher diagnosis rate in terms of clinical diagnosis of tracheal for-eign body with very positive application value.
