目的:研究超声实时弹性成像在鉴别甲状腺良恶性结节的效果及应用价值。方法随机在该院选择200例(232个结节)患者有甲状腺疾病的患者,采用常规超声及超声实时弹性成像两种方法检测结节的良恶性,在两种鉴别方法鉴别之后,根据最后病理结果将结节分为良恶性两组,并将结果与两种方法检测结果对比,了解超声实时弹性成像在诊断甲状腺结节良恶性中的价值。结果根据所得结果可知,按照弹性分级以≤Ⅱ级为良性结节,弹性分级以≥Ⅲ级为恶性结节,超声实时弹性成像在鉴别良恶性结节的准确率为90.95%,常规超声的准确率为62.93%,相比差异有统计学意义(P<0.05)。超声弹性分级诊断的敏感度﹑特异度分别为85.19%﹑94.04%,与常规组的26.51%﹑77.99%相比差异有统计学意义(P<0.05)。结论超声实时弹性成像诊断甲状腺良恶性结节准确性高,此技术在临床上可减少对病理检测的依赖,减轻患者痛苦,无创的鉴别良恶性,可推荐。

Objective To study the effect and application value of the real-time ultrasound elastography in differentiating benign and malignant thyroid nodules. Methods 200 patients with thyroid disease (232 nodules) admitted in our hospital were randomly selected. The conventional ultrasound and real-time ultrasound elastography were used to differentiate benign and malignant thy-roid nodules. And the nodules were divided into benign group and malignant group according to the pathological results. The pathological results of the nodules were compared with the detection results of the conventional ultrasound and real-time ultra-sound elastography so as to evaluate the value of real-time ultrasound elastography in the differential diagnosis of benign from ma-lignant thyroid nodules. Results According to the elastic grade, thyroid nodules≤II were benign,≥Ⅲwere malignant. The results showed that the accuracy of real-time ultrasound elastography in differentiating benign from malignant nodules was 90.95%, and that of the conventional ultrasound was 62.93%, the difference between them was statistically significant (P<0.05). The sensitivity and specificity of ultrasound elastography was 85.19%, 94.04%, respectively, and that of conventional ultrasound was 26.51%, 77.99%, respectively, the difference between them was statistically significant (P<0.05). Conclusion Real-time ultrasound elastography has high accuracy in the differential diagnosis of benign and malignant thyroid nodules, which can reduce the patients' dependence on pathological examination, alleviate the suffering of the patients with non-invasive identification, so it can be recommended.
