目的：探讨DHS和PFNA固定治疗老年股骨粗隆间骨折的疗效。方法：2012年9月-2013年6月收治老年股骨粗隆间骨折患者50例，随机分为对照组和观察组各25例，观察组采用DHS治疗，对照组采用PFEN治疗，观察两组的治疗效果。结果：经治疗，观察组的手术时间、术中出血量、负重时间以及骨折愈合时间等手术相关指标明显优于对照组，术后 Harris 疗效优良率明显高于对照组(92.0% VS 64.0%)，差异有统计学意义(P＜0.05)。结论：与 DHS 相比，PFNA固定法治疗老年股骨粗隆间骨折，治疗有效率与安全性都比较高，值得临床推广。

목적：탐토DHS화PFNA고정치료노년고골조륭간골절적료효。방법：2012년9월-2013년6월수치노년고골조륭간골절환자50례，수궤분위대조조화관찰조각25례，관찰조채용DHS치료，대조조채용PFEN치료，관찰량조적치료효과。결과：경치료，관찰조적수술시간、술중출혈량、부중시간이급골절유합시간등수술상관지표명현우우대조조，술후 Harris 료효우량솔명현고우대조조(92.0% VS 64.0%)，차이유통계학의의(P＜0.05)。결론：여 DHS 상비，PFNA고정법치료노년고골조륭간골절，치료유효솔여안전성도비교고，치득림상추엄。
Objective:To explore the effects of DHS and PFNA fixation in the treatment of intertrochanteric fracture of femur in the elderly.Methods:50 elderly patients with femoral intertrochanteric fracture were selected from September 2012 to June 2013. They were randomly divided into the control group and the observation group with 25 cases in each group.The observation group were treated with DHS,while the control group were treated with PFEN.We observed the effect of treatment of the two groups. Results:After the treatment,in the observation group,operation time,amount of bleeding during operation,weight loading time and the time of fracture healing were significantly better than the control group;after operation,the excellent and good rate of Harris was significantly higher than that of control group(92% VS 64%);the differences were statistically significant(P<0.05).Conclusion:Compared with DHS,PFNA fixation method was used for the treatment of intertrochanteric fracture of femur in the elderly,and the effective rate of the treatment and safety were relatively high.It was worth the clinical promotion.
