目的：探讨头位难产的发生原因及其临床处理技术。方法：2012年3月-2014年3月收治头位难产患者294例，对其临床资料进行回顾性分析。结果：产妇产道异常35例(11.9%)，胎头位置异常220例(74.8%)，产力异常(即宫缩乏力)39例(13.3%)；实施剖宫产211例(71.8%)，实施阴道分娩83例(28.2%)。结论：在临床上一旦发现孕产妇存在头位难产，应依据孕产妇的实际状况选取恰当的处理方式，以便最大限度地降低母婴死亡率。

목적：탐토두위난산적발생원인급기림상처리기술。방법：2012년3월-2014년3월수치두위난산환자294례，대기림상자료진행회고성분석。결과：산부산도이상35례(11.9%)，태두위치이상220례(74.8%)，산력이상(즉궁축핍력)39례(13.3%)；실시부궁산211례(71.8%)，실시음도분면83례(28.2%)。결론：재림상상일단발현잉산부존재두위난산，응의거잉산부적실제상황선취흡당적처리방식，이편최대한도지강저모영사망솔。
Objective:To discuss the causes and clinical treatment technology of head presentation dystocia.Methods:294 patients with head presentation dystocia were selected from March 2012 to March 2014.We analyzed the clinical data.Results:Maternal birth canal abnormalities in 35 cases(11.9%),abnormal fetal head position in 220 cases(74.8%),productivity abnormal(ie uterine inertia)in 39 cases(13.3%);the implementation of cesarean section in 211 cases(71.8%),implementation of vaginal delivery in 83 cases (28.2%).Conclusion:Once the clinical presence of maternal dystocia,appropriate treatment should be selected based on the actual situation of maternal,in order to maximize the reduction of maternal and infant mortality.
