目的：观察高压氧联合卒中单元对脑出血偏瘫患者运动功能及抑郁状态的影响。方法：选取脑出血患者，按照数字随机法分为观察组和对照组，对照组采用常规内科治疗，观察组按照卒中单元标准流程并接受高压氧治疗。随访3个月，采用Barthel指数评估日常生活能力(ADL)，Fugl-Meyer运动功能积分(FMA)评估运动功能和HAMD量表评估抑郁状态，比较两组患者治疗前后ADL、FMA和HAMD评分的差异。结果：3个月后与治疗前比较，两组患者的ADL和FMA评分均有不同程度提高，HAMD评分均有降低，差异具有统计学意义(P＜0.05)；3个月后组间比较，观察组ADL与FMA评分均显著高于对照组，而观察组HAMD评分低于对照组，差异具有统计学意义(P＜0.05)。观察组和对照组组住院期间并发症及脑出血后遗症分别为21.4%和50.0%，差异具有统计学意义(P＜0.05)。结论：高压氧联合卒中单元可改善脑出血偏瘫患者运动功能、抑郁状态，降低后期并发症。
목적：관찰고압양연합졸중단원대뇌출혈편탄환자운동공능급억욱상태적영향。방법：선취뇌출혈환자，안조수자수궤법분위관찰조화대조조，대조조채용상규내과치료，관찰조안조졸중단원표준류정병접수고압양치료。수방3개월，채용Barthel지수평고일상생활능력(ADL)，Fugl-Meyer운동공능적분(FMA)평고운동공능화HAMD량표평고억욱상태，비교량조환자치료전후ADL、FMA화HAMD평분적차이。결과：3개월후여치료전비교，량조환자적ADL화FMA평분균유불동정도제고，HAMD평분균유강저，차이구유통계학의의(P＜0.05)；3개월후조간비교，관찰조ADL여FMA평분균현저고우대조조，이관찰조HAMD평분저우대조조，차이구유통계학의의(P＜0.05)。관찰조화대조조조주원기간병발증급뇌출혈후유증분별위21.4%화50.0%，차이구유통계학의의(P＜0.05)。결론：고압양연합졸중단원가개선뇌출혈편탄환자운동공능、억욱상태，강저후기병발증。
Objective:To investigate the effects of hyperbaric oxygen combined with stroke unit on motor function and depression status of patients with hemiplegic cerebral hemorrhage.Methods:Patients with cerebral hemorrhage were selected.They were randomly divided into the observation group and the control group.The patients of the control group were treated with conventional medical,and the observation group received hyperbaric oxygen therapy according to the standard process of stroke unit.Follow up of 3 months,with the Barthel index to evaluate the ability of daily life(ADL),Fugl-Meyer motor function integral(FMA)assessment of motor function and HAMD scale to assess depressive state.The differences of ADL,FMA and HAMD scores were compared before and after treatment of patients in the two groups.Results:The two groups of patients with ADL score and FMA score were improved in different degree,HAMD scores were lower,with significant difference after 3 months compared with before treatment(P<0.05).Comparison between groups after 3 months,the ADL and FMA scores of the observation group were significantly higher than the control group,while HAMD scores of the observation group were lower than those of the control group, with significant difference(P<0.05).The complications and sequelae of cerebral hemorrhage were 21.4% and 50% during the period of hospitalization of the observation group and the control group,the difference was statistically significant(P<0.05). Conclusion:Hyperbaric oxygen combined with stroke unit can improve the motor function and depression status of patients with hemiplegic cerebral hemorrhage and reduce late complications.
