目的:探讨原发性高血压患者肾脏早期损害的诊断方法。方法:2011年4月-2012年8月收治高血压病患者181例,高血压肾病组(DN组)83例和高血压非肾病组(NDN组)98例,另设健康对照组(N组)100例。采用免疫散射比浊法测定血清胱抑素C(CYS-C)、β2微球蛋白(β2-MG)、速率法测定血清尿素(UREA)、酶法测定肌酐(CR)、尿酸酶法测定血清尿酸(UA)。结果:83例原发性高血压患者肾损伤组(DN)CYS-C明显高于98例非高血压患者肾损伤组(NDN)和正常对照组(P<0.01);同时测定β2-MG可提高原发性高血压患者肾病早期损害的异常检出率(90%)。结论:CYS-C在临床上可作为肾小球滤过率(GFR)的判断指标,有助于高血压肾病的早期诊断。

목적:탐토원발성고혈압환자신장조기손해적진단방법。방법:2011년4월-2012년8월수치고혈압병환자181례,고혈압신병조(DN조)83례화고혈압비신병조(NDN조)98례,령설건강대조조(N조)100례。채용면역산사비탁법측정혈청광억소C(CYS-C)、β2미구단백(β2-MG)、속솔법측정혈청뇨소(UREA)、매법측정기항(CR)、뇨산매법측정혈청뇨산(UA)。결과:83례원발성고혈압환자신손상조(DN)CYS-C명현고우98례비고혈압환자신손상조(NDN)화정상대조조(P<0.01);동시측정β2-MG가제고원발성고혈압환자신병조기손해적이상검출솔(90%)。결론:CYS-C재림상상가작위신소구려과솔(GFR)적판단지표,유조우고혈압신병적조기진단。
Objective:To discuss the diagnostic method of kidney early damage in patients with primary hypertension.Methods:181 cases ofprimary hypertension were selected from April 2011 to August 2012,including 83 cases of hypertensive nephropathy(DN group)and 98 cases of non hypertensive nephropathy (NDN group).100 cases of health control group(N group) were selected.The serum cystatin C(CYS-C) andβ 2-MG were measured by immune scatter turbidity;the serum urea was measured by velocity method;the creatinine(CR) was measured by enzymic method;the serum uric acid(UA) was measured by uric acid enzymatic method.Results:CYS-C of the kidney injury of patients with primary hypertension group(DN) in 83 cases was significantly higher than that of no kidney injury of patients with hypertension group(NDN) and normal control group(P<0.01).At the same time measuringβ2-MG can improve the anomaly detection rate(90%) of the nephropathy early damage indexes in patients with primary hypertension.Conclusion:CYS-C can be used as the judgment index of the glomerular filtration rate(GFR) in the clinical,helpful to the early diagnosis of hypertensive nephropathy.
