目的：探讨临床上采用多巴酚丁胺联合硝酸甘油、速尿在治疗顽固性心衰中的疗效。方法回顾性分析该院2009年11月—2014年11月收治的顽固性心衰患者的临床资料。治疗组采用多巴酚丁胺联合硝酸甘油和速尿用药治疗，对照组采用洋地黄制剂进行治疗。比较分析两组患者治疗前后的心率、血压和临床疗效。结果治疗组患者治疗前后心率、收缩压和舒张压差异有统计学意义(<0.05)；而对照组患者治疗前后心率、收缩压和舒张压差异无统计学意义(>0.05)；治疗组患者总有效率明显高对照组，总有效分别为94.1%和89.1%，差异有统计学意义(<0.05)。结论多巴酚丁胺联合硝酸甘油、速尿联合治疗能够快速控制心率和血压，总的疗效较好，有临床应用和推广价值。
목적：탐토림상상채용다파분정알연합초산감유、속뇨재치료완고성심쇠중적료효。방법회고성분석해원2009년11월—2014년11월수치적완고성심쇠환자적림상자료。치료조채용다파분정알연합초산감유화속뇨용약치료，대조조채용양지황제제진행치료。비교분석량조환자치료전후적심솔、혈압화림상료효。결과치료조환자치료전후심솔、수축압화서장압차이유통계학의의(<0.05)；이대조조환자치료전후심솔、수축압화서장압차이무통계학의의(>0.05)；치료조환자총유효솔명현고대조조，총유효분별위94.1%화89.1%，차이유통계학의의(<0.05)。결론다파분정알연합초산감유、속뇨연합치료능구쾌속공제심솔화혈압，총적료효교호，유림상응용화추엄개치。
Objective To investigate the clinical use of dobutamine combined with nitroglycerin, furosemide in the treatment of intractable heart failure in effect. Methods Retrospective analysis of the clinical data of our hospital from 2009 November to 2014 November were refractory heart failure patients. The treatment group used dobutamine combined with nitroglycerin and furosemide therapy in the treatment, control group were treated with digitalis. Comparative analysis of heart rate, blood pressure and the clinical curative effect of two groups of patients before and after treatment. Results Before and after treatment for the patients with heart rate, systolic and diastolic blood pressure difference is significant ( <0.05);while the control group before and after treatment in patients with heart rate, systolic and diastolic blood pressure difference is not significant ( >0.05);the patients in the treatment group total effective rate was obviously higher in control group, the total effective were 94.1%and 89.1%, the difference was significant ( <0.05). Conclusion Dobutamine combined with nitroglycerin, furosemide combined therapy can quickly control the heart rate and blood pressure, the better the total curative effect, clinical application and popularization value.
