目的：研究高位复杂肛瘘使用不同手术治疗的临床效果。方法临床纳入80例该院2012年8月-2014年8月期间收治的高位复杂肛瘘患者，将所有患者按照随机抽样法分为两组各40例。其中40例患者使用外切内挂胶管引流术进行治疗作为观察组，另40例患者使用切开挂线术进行治疗作为对照组。结果观察组治疗总有效率高达100%，复发率仅为2.5%，远好于对照组的95%、15%，P<0.05；而术后住院时间、伤口愈合时间及住院费用，观察组均优于对照组，P<0.05。结论外切内挂胶管引流术治疗高位复杂肛瘘效果显著，预后良好，术后复发率低，且费用较低，值得临床应用及推广。
목적：연구고위복잡항루사용불동수술치료적림상효과。방법림상납입80례해원2012년8월-2014년8월기간수치적고위복잡항루환자，장소유환자안조수궤추양법분위량조각40례。기중40례환자사용외절내괘효관인류술진행치료작위관찰조，령40례환자사용절개괘선술진행치료작위대조조。결과관찰조치료총유효솔고체100%，복발솔부위2.5%，원호우대조조적95%、15%，P<0.05；이술후주원시간、상구유합시간급주원비용，관찰조균우우대조조，P<0.05。결론외절내괘효관인류술치료고위복잡항루효과현저，예후량호，술후복발솔저，차비용교저，치득림상응용급추엄。
Objective To study the clinical effects of different surgical options for high complex anal fistula. Methods 80 patients with higher complex anal fistula treated from August 2012 to August 2014 in our hospital were selected and randomly divided into two groups, 40 cases each group. The observation group received the circumscribed drainage combined with hose drainage inside while control group received the incision-thread-drawing procedure. Results The total effective rate and recurrence rate of obser-vation group were 100% and 2.5% while the respective rates of control group were 95% and 15% (P<0.05). LOS after surgery, time of wound healing and hospitalization costs of observation group were better than those of control group (P<0.05). Conclusion The circumscribed drainage combined with hose drainage inside has an obvious treatment effect for high complex anal fistula with favorable prognosis and lower recurrence rate and costs, which is worthy of clinical application and promotion.
