目的：探讨鼻内镜下与传统手术治疗鼻中隔偏曲的临床疗效。方法选择整群2007年3月-2013年10月在该院行鼻内镜下鼻中隔偏曲矫正术的57例患者作为内镜组，另选取同期行传统鼻中隔矫正术的55例患者作为传统组，比较两组手术时间、术中失血量、术后住院时间、术后24 h患者疼痛情况、手术相关并发症及术前和术后鼻塞症状VAS评分。结果与传统组相比较，内镜组患者手术时间和术后住院时间均明显缩短，术中失血量明显减少(P<0.05)；内镜组患者术后24 h疼痛程度明显轻于传统组(P<0.05)；经手术后，两组患者鼻塞症状VAS评分均较术前有了显著改善(P<0.05)；但与传统组相比较，内镜组改善程度更为明显(P<0.05)；两组总有效率比较差异无统计学意义(P>0.05)，但内镜组治愈率明显高于传统组(P<0.05)；内镜组不良反应发生率为3.51%，明显低于传统组的14.55%(P<0.05)。结论鼻内镜手术与传统手术均能有效治疗鼻中隔偏曲，但鼻内镜手术耗时更短、创伤更小、术后恢复更快、疼痛更轻，且并发症少，值得临床推广应用。
목적：탐토비내경하여전통수술치료비중격편곡적림상료효。방법선택정군2007년3월-2013년10월재해원행비내경하비중격편곡교정술적57례환자작위내경조，령선취동기행전통비중격교정술적55례환자작위전통조，비교량조수술시간、술중실혈량、술후주원시간、술후24 h환자동통정황、수술상관병발증급술전화술후비새증상VAS평분。결과여전통조상비교，내경조환자수술시간화술후주원시간균명현축단，술중실혈량명현감소(P<0.05)；내경조환자술후24 h동통정도명현경우전통조(P<0.05)；경수술후，량조환자비새증상VAS평분균교술전유료현저개선(P<0.05)；단여전통조상비교，내경조개선정도경위명현(P<0.05)；량조총유효솔비교차이무통계학의의(P>0.05)，단내경조치유솔명현고우전통조(P<0.05)；내경조불량반응발생솔위3.51%，명현저우전통조적14.55%(P<0.05)。결론비내경수술여전통수술균능유효치료비중격편곡，단비내경수술모시경단、창상경소、술후회복경쾌、동통경경，차병발증소，치득림상추엄응용。
Objective To explore the clinical curative effect of nasal septum deviation and traditional surgery under nasal endo-scope. Methods Selected 57 cases from March 2007 to October 2013 in our hospital endoscopic nasal septum surgery patients as the endoscopic group, the other 55 cases selected underwent traditional nasal septum surgery patients as the traditional group, op-eration time, intraoperative blood loss, compared with two groups of postoperative hospital stay, postoperative pain 24 h patients, complications related to operation and preoperative and postoperative VAS scores of nasal obstruction symptoms. Results Com-pared with the traditional group, the endoscopic group of patients, operation time and postoperative hospital stay were significantly shortened, the blood loss in operation were significantly reduced(P<0.05);endoscopic groups of patients with 24 h pain were signif-icantly lighter than the conventional group (P<0.05);after the operation, two groups of nasal symptoms in patients with VAS scores compared with the preoperative have significantly improved (P<0.05); compared with the traditional group, the endoscopic group improved more obviously(P<0.05);two group there were no statistically significant differences in efficiency comparison(P>0.05), but the endoscopic group the cure rate significantly higher than the traditional group (P<0.05); endoscopic group the incidence of ad-verse reaction was 3.51%, significantly lower than that of the traditional group 14.55%(P<0.05). Conclusion Endoscopic surgery and conventional surgery was effective in the treatment of deviation of nasal septum, nasal endoscopic surgery but consumes less time, less trauma, faster postoperative recovery, lighter pain, and less complications, which is worthy of clinical application.
