目的：探讨小骨窗微创脑出血清除术治疗高血压脑出血的临床效果。方法：收治高血压脑出血患者2000例，根据患者的治疗方式分为 A、B、C 3组，A组620例行传统大骨瓣开颅手术，B组645例行头颅血肿穿刺抽吸治疗，C组行735例行小骨窗微创脑出血清除术治疗。结果：C组总有效率、血肿清除率、术后再出血率、并发症率及病死率均显著优于A、B两组(P＜0.01)。结论：小骨窗微创脑出血清除术在高血压脑出血治疗中效果显著优于传统大骨瓣开颅手术与头颅血肿穿刺抽吸治疗。

목적：탐토소골창미창뇌출혈청제술치료고혈압뇌출혈적림상효과。방법：수치고혈압뇌출혈환자2000례，근거환자적치료방식분위 A、B、C 3조，A조620례행전통대골판개로수술，B조645례행두로혈종천자추흡치료，C조행735례행소골창미창뇌출혈청제술치료。결과：C조총유효솔、혈종청제솔、술후재출혈솔、병발증솔급병사솔균현저우우A、B량조(P＜0.01)。결론：소골창미창뇌출혈청제술재고혈압뇌출혈치료중효과현저우우전통대골판개로수술여두로혈종천자추흡치료。
Objective:To investigate the clinical effect of the minimal invasive brain hemorrhage dissection in the treatment of hypertensive cerebral hemorrhage.Methods:2 000 patients with hypertensive cerebral hemorrhage were selected.They were divided into the A, B, C groups according to the treatment.620 cases of the group A were treated with traditional large bone flap craniotomy.645 cases of group B were given hematoma aspiration treatment.735 cases of the group C were received minimal invasive brain hemorrhage dissection.Results:The total effective rate,hematoma clearance rate,postoperative bleeding rate, complication rate and the mortality rate of the C group were significantly higher than those of the A and the B group(P<0.01). Conclusion:The curative effect of minimal invasive brain hemorrhage dissection in the treatment of hypertensive cerebral hemorrhage was significantly better than that of the traditional large bone flap craniectomy and hematoma aspiration treatment.
