目的：探讨比较应用不同材料支架义齿修复下牙列缺损的临床效果。方法以2008-2014年在该院进行可摘局部义齿术的1200例下牙列缺损患者为研究对象，义齿共1452个。按照患者选择的支架义齿材料的不同，将患者分为纯钛组、钴铬合金组和Vitallium2000组。修复前，检查和记录患者的口腔情况，修复后，比较3组患者义齿就位率、2W调整率、义齿性口炎发生率、咀嚼效率、不良症状发生情况以及患者对义齿各个方面的满意度。结果与纯钛组与钴铬合金组相比，Vital-lium2000组患者义齿就位率和咀嚼效率较高，2W调整率和义齿性口炎发生率较低，差异有统计学意义（P<0.01）；较少发生咀嚼功能不佳、发音不良等不良症状，阳性率较低，差异有统计学意义（P<0.05）；在美观度、舒适度等方面满意度更高（P<0.05）。结论与纯钛支架和钴铬合金支架相比，Vitatlium2000支架义齿更为轻巧稳固，美观度和舒适度更高，患者咀嚼功能良好，患者的2W调整率以及义齿性口炎率降低，患者满意度提高。
목적：탐토비교응용불동재료지가의치수복하아렬결손적림상효과。방법이2008-2014년재해원진행가적국부의치술적1200례하아렬결손환자위연구대상，의치공1452개。안조환자선택적지가의치재료적불동，장환자분위순태조、고락합금조화Vitallium2000조。수복전，검사화기록환자적구강정황，수복후，비교3조환자의치취위솔、2W조정솔、의치성구염발생솔、저작효솔、불량증상발생정황이급환자대의치각개방면적만의도。결과여순태조여고락합금조상비，Vital-lium2000조환자의치취위솔화저작효솔교고，2W조정솔화의치성구염발생솔교저，차이유통계학의의（P<0.01）；교소발생저작공능불가、발음불량등불량증상，양성솔교저，차이유통계학의의（P<0.05）；재미관도、서괄도등방면만의도경고（P<0.05）。결론여순태지가화고락합금지가상비，Vitatlium2000지가의치경위경교은고，미관도화서괄도경고，환자저작공능량호，환자적2W조정솔이급의치성구염솔강저，환자만의도제고。
Objective To discuss and compare the clinical effects of frame denture made of different materials for the repair of lower dentition defect. Methods 1200 cases with lower dentition defect underwent removable partial denture surgery (1452 den-tures) in our hospital from 2008 to 2014 were selected as the subjects and divided into the titanium group, cobalt-chromium alloy group and Vitallium 2000 group in accordance with the material of framework denture selected by the patients. Before the repair, the conditions of the oral cavity of the patients were examined and recorded. After repair, the rate of denture in position, 2W regu-lation rate, incidence of denture stomatitis, chewing efficiency, incidence of adverse symptoms and patient satisfaction with various aspects of the denture of the three groups were compared. Results Compared with titanium and cobalt-chromium alloy group, Vi-tallium 2000 group had much higher rate of denture in position, chewing efficiency and obvious lower 2W regulation rate and inci-dence of denture stomatitis,The difference was statistically significant(P<0.01);significant less poor chewing function and poor pro-nunciation and other adverse symptoms and lower positive rate,The difference was statistically significant(P<0.05);and much higher satisfaction with the denture appearance and comfort and so on,The difference was statistically significant (P<0.05).Conclusion Compared with titanium and cobalt-chromium alloy framework denture, Vitallium 2000 framework denture is lighter, solider, pret-tier and more comfortable with higher chewing efficiency and lower 2W regulation rate and incidence of denture stomatitis, patients are more satisfied with it.
