目的:探讨床旁检测B型钠尿肽在鉴别急性呼吸困难中的诊断价值。方法:将150例呼吸困难患者分为肺源性呼吸困难60例和心源性呼吸困难90例,所有患者均进行床旁检查 BNP。结果:心源性呼吸困难组 BNP(820.2±450.2)pg/mL,LVEF(43±10)%;肺源性呼吸困难组BNP(212.3±175.3)pg/mL,LVEF(60±8)%,两组比较,差异有统计学意义(P<0.05)。心功能分级Ⅱ级 BNP(410.2±189.2)pg/mL, LVEF(53±6)%,Ⅲ级 BNP(860.2±289.2)pg/mL, LVEF(41±10)%,Ⅳ级BNP(1162.3±465.3)pg/mL,LVEF(36±8)%,BNP与心功能分级呈正相关,LVEF与心功能分级负正相关。结论:BNP检测具有快速、高灵敏度、高特异性的特点,是鉴别心源性呼吸困难和肺源性呼吸困难的重要指标。

Objective:To explore the diagnostic value of bedside testing b-type natriuretic peptide in the differential diagnosis of acute dyspnea.Methods:150 patients with dyspnea were divided into pulmonary dyspnea with 60 cases and cardiac dyspnea with 90 cases.All patients were carried out by bedside testing BNP.Results:BNP was (820.2±450.2)pg/mL,LVEF was (10±43)% in the cardiac dyspnea group.BNP was (212.3±175.3)pg/Ml,LVEF was (60±8)% in the pulmonary dyspnea.The differences of two groups were statistically significant(P<0.05).BNP was (410.2 ± 189.2)pg/mL,LVEF was (53 ± 6)% in the cardiac functional grading Ⅱgrade.BNP was (860.2±289.2)pg/mL,LVEF was (41±10)% of Ⅲ grade.BNP was (1162.3±465.3)pg/mL,LVEF was (36±8)% of Ⅳ grade.BNP was positively correlated with cardiac functional grading,and LVEF was negatively correlated with cardiac functional grading.Conclusion:BNP detection has the characteristics of rapid,high sensitivity,high specificity.It is an important indicator for the differential diagnosis of cardiac dyspnea and pulmonary dyspnea.
