目的 观察妊娠期高血压疾病患者静脉血N端脑钠肽前体( NT-proBNP)水平的变化,探讨床旁快速检测NT-proBNP对妊娠期高血压疾病病情判断的价值. 方法 选择妊娠期高血压疾病患者69例(其中妊娠高血压组16例、子痫前期组30例、子痫组23例)及与其年龄匹配正常妊娠组30例. 入ICU第1天计算各组急性生理学及慢性健康状况评分系统Ⅱ( APACHEⅡ)评分,记录收缩压、舒张压、氧合指数、左室射血分数( LVEF);入ICU的第1、3、5天应用床旁快速检测仪检测各组静脉血NT-proBNP水平,分析NT-proBNP水平变化及其与病变严重程度的关系. 结果 妊娠期高血压疾病三组APACHEⅡ评分、收缩压、舒张压、氧合指数、LVEF比较均有统计学差异( P均<0.01). 子痫组入ICU第1天NT-proBNP水平高于子痫前期组、妊娠高血压组(P均<0.05),经过治疗,随病情好转各组NT-proBNP水平均降低,入ICU第5天与第1天比较差异有统计学意义(P均<0.05). 妊娠期高血压疾病患者入ICU第1天APACHEⅡ评分与NT-proBNP水平呈正相关(r=0.752,P<0.01). 结论 床旁快速检测NT-proBNP水平对妊娠期高血压疾病患者病情判断有重要价值.

목적 관찰임신기고혈압질병환자정맥혈N단뇌납태전체( NT-proBNP)수평적변화,탐토상방쾌속검측NT-proBNP대임신기고혈압질병병정판단적개치. 방법 선택임신기고혈압질병환자69례(기중임신고혈압조16례、자간전기조30례、자간조23례)급여기년령필배정상임신조30례. 입ICU제1천계산각조급성생이학급만성건강상황평분계통Ⅱ( APACHEⅡ)평분,기록수축압、서장압、양합지수、좌실사혈분수( LVEF);입ICU적제1、3、5천응용상방쾌속검측의검측각조정맥혈NT-proBNP수평,분석NT-proBNP수평변화급기여병변엄중정도적관계. 결과 임신기고혈압질병삼조APACHEⅡ평분、수축압、서장압、양합지수、LVEF비교균유통계학차이( P균<0.01). 자간조입ICU제1천NT-proBNP수평고우자간전기조、임신고혈압조(P균<0.05),경과치료,수병정호전각조NT-proBNP수평균강저,입ICU제5천여제1천비교차이유통계학의의(P균<0.05). 임신기고혈압질병환자입ICU제1천APACHEⅡ평분여NT-proBNP수평정정상관(r=0.752,P<0.01). 결론 상방쾌속검측NT-proBNP수평대임신기고혈압질병환자병정판단유중요개치.
Objective To observe the changes in levels of venous blood N-terminal pro-brain natriuretic peptide ( NT-proBNP) of patients with gestational hypertension, and to explore the diagnostic values of bedside rapid detection of NT-proB-NP.Methods We selected 69 patients with gestational hypertension, including the gestational hypertension group (n=16), the preeclampsia group (n=30) and the eclampsia group (n=23) and another 30 age-matched cases of normal pregnancy women.Within 24 hours after their entering to the ICU, the acute physiology and chronic health evaluation ( APACHE II) scores were evaluated;the systolic pressure, the diastolic pressure, the oxygenation index and LVEF were recorded.On the first, the third and the fifth day of their entering to the ICU, bedside rapid detection technololy was used to detect the NT-proBNP in venous blood to analyze the changes of NT-proBNP in each group and the severity of the lesion.Results There were statistical differences in the APACHE II scores, the systolic pressure, the diastolic pressure, the oxygenation index and LVEF among thees three groups (all P<0.01).On the first day of their entering the ICU, the NT-proBNP level of the ec-lampsia group was higher than those of the preeclampsia group and the gestational hypertension group (all P<0.05).After the treatment, the level of NT-proBNP of each group was decreased, and there was statistically significant different in the re-sult on the fifth day as compared with that of the first day (all P<0.05).The APACHEⅡscore on the first day of entering ICU for the gestational hypertension group was positively correlated with the NT-proBNP level (r=0.752, P<0.01).Con-clusion Bedside rapid detection of NT-proBNP is of great importance for the diagnosis of gestational hypertension.
