目的：选择高血压基底节区脑出血外科最佳手术方案，提高救治成功率。方法：评估本院高血压脑出血患者开颅治疗的预后，结合患者条件，对个体化选择手术时机和术式的疗效进行回顾性分析。结果：小骨窗患者186例，术后复发出血者者4例，并发较严重硬膜下积液者4例。结论：与许多文献报道相反，临床证实小骨窗开颅治疗高血压脑出血预后欠佳，不值得推荐。

목적：선택고혈압기저절구뇌출혈외과최가수술방안，제고구치성공솔。방법：평고본원고혈압뇌출혈환자개로치료적예후，결합환자조건，대개체화선택수술시궤화술식적료효진행회고성분석。결과：소골창환자186례，술후복발출혈자자4례，병발교엄중경막하적액자4례。결론：여허다문헌보도상반，림상증실소골창개로치료고혈압뇌출혈예후흠가，불치득추천。
Objective:To select the best surgical treatment for hypertensive cerebral hemorrhage in basal ganglia region,and to improve the success rate of treatment.Methods:To evaluate the prognosis of patients with hypertensive cerebral hemorrhage in our hospital,with patient conditions,a retrospective analysis was made on the effect of the choice of individual operation and the operation time.Results:Small bone window in 186 cases,4 cases of recurrence of postoperative bleeding,4 cases complicated with severe subdural effusion.Conclusion:Choosing contrary to many reports in the literature,clinically proven small bone window craniotomy for treatment of hypertensive cerebral hemorrhage prognosis is poor,not worthy of recommendation.
